2. Who Should Use Form 7?

You need to apply using this form if:

    • A ration card member is a minor (under 18) or legally incapable (mentally/physically).
    • That member does not have a natural guardian and requires an appointed guardian for scheme access.
    • The guardian will conduct all ration-related tasks, including obtaining supplies, making changes, and official communication.

3. Required Documents

Be sure to gather the following:

    1. Form 7, duly filled (available for download from Digital Gujarat or Jan Seva Kendra).
    2. Proof of minor’s identity: Birth certificate / school record / existing ration card extract listing the minor.
    3. Guardian’s identity: Aadhaar, voter ID, or PAN.
    4. Proof of relationship: Birth certificate, affidavit, or family card.
